following forms can be downloaded in either a PDF format or a
Microsoft Word format. The Word format allows you to complete
the forms electronically.
Cheer Fusion Waiver Forms
Waiver forms are required to participate in any Cheer
Fusion activity including classes, open gym, camps, clinics or competitive teams.
Waiver forms must be signed by a parent or guardian of the
participating child prior to participation.

Cheer Fusion Member Registration Forms
Registration forms are required for new Cheer Fusion
class, private lesson, or team members. Registration forms must be completed
entirely; including all requested insurance information and
presented by the first day of activity.

Competitive Team Referral Form
Cheer Fusion's referral program gives new and
existing competitive team members an opportunity to earn $50.00 for
each qualifying referral.
